Summary of Position:
The Gizhewaadiziwin Health Access Centre is currently accepting
applications for a contract System Navigator/OTN Coordinator. This
position is a permanent full-time position. The System Navigator/OTN
Coordinator will act part of the clinical interdisciplinary team.
Duties and Responsibilities:
This position will require assisting patients with the coordination of their
wholistic healthcare throughout the organization and the healthcare
system. This will include consulting with the healthcare team on the
needs of patients and referrals to other programs and specialists, as well
as support and education to the patients and their families around illness
and disease diagnosis and treatments. The position will also include
the coordination of OTN (The Ontario Telemedicine Network) patient
appointment, the provision of support to patients involved in OTN
appointments, and operation of the OTN equipment.

  • Graduate from an accredited School of Nursing;
  • Current Ontario RPN License;
  • Good standing with the College of Nurses;
  • Excellent written and verbal communication skills;
  • Ability to work as a team member;
  • 2 - 3 years clinic nursing experience;
  • Previous experience/training on the OTN;
  • Experience and familiarity with community and specialist services

available both locally and regionally;

  • Valid Driver’s License.


  • Fluency in the Ojibway language, a definite asset
  • Prior experience working with aboriginal people and cultural

safety training

  • Current CPR certification
