Riverside Health Care Facilities

(1 Year Contract)
In this temporary contract position, the Project Coordinator works
collaboratively in supporting Rainy River District health care
agencies in the establishment and operationalizing of the Health
Links approach to care. The Project Coordinator is accountable to
RHCF and the Rainy River District Health Links Steering Committee
in accordance with the business plan and established timelines.
The Health Links Project Coordinator works with participating
partners with an overall goal to improve quality of care and service
coordination for persons dealing with complex chronic health and
social conditions.
